A Child With Fits and Hemiparesis
Labels: Interesting Medicine
I presented this rare but interesting case during yesterday's ward round: a 20-month old toddler who came to the hospital with left-sided hemiparesis (weakness). He had two episodes of focal seizures at home, involving his left upper and lower limbs, during which there was no lost of consciousness. He had no fever.
He developed weakness on the left side of his body after the seizures. An obvious physical finding, which is very much related to his seizures, is the purplish "birthmark" (he was born with it) on his face, medically termed "port-wine stain". Can you guess the diagnosis?

Epilepsy lah! what has his birthmark to do with his seizures? trick question ah?
Wah spasticity man!Does he has difficulty with motor control, some are sterotypic la.cerebral palsy or could be SW syndrome?
But im not sure whether the vascular birthmark has anything to do with his seizures
Wah all medical terms some out...faint
[zac]:
it's not epilepsy. his birthmark does not cause his seizures, but is related to it. it's part of a syndrome. read on for the answer! :)

bravo Dr. Ray, spot-on diagnosis! Sturge-Weber Syndrome (SWS) it is. Port-wine stain is venule ectasia, and cause of seizure is ischaemic brain injury secondary to overlying meningeal angioma. he's having some kind of stroke in evolution.
[steve]:
actually there's a big hint there. if you google "port-wine stain", you'll have the diagnosis too!
i don't even know what is "port-wine stain" future Dr. Lim. This of course didn't come across my mind hence i won't goggle something that i dunno rite. Anyway thanks for enlightening me...haha
Wow! All these doctor talk. Sounds so impressive and sophisticated :) Me can't diagnose. *creeps quietly away*
[steve]:
"port-wine stain" is a lay term, meaning stained in the colour of port wine, which is purplish in colour. poor toddler, Steve not interested to know what he's suffering from! haha.
[poohbearee]:
